Waivers for registration

Cancellation Policy

Refunds may be given as follows: Full Refund 60 days prior to the first day of camp. 75% Refund 45 days prior to first day of camp. 50% Refund 15 days prior to the first day of camp. After June 1st , which is 5 days before the start of camps, there will be no refunds offered unless under extenuating circumstances, and will be reviewed in a case by case manner. 
 

Consent to Treatment

I hereby authorize physicians, nurses, hospitals, and their authorized personnel, whether or not they are employed, contracted, or paid on a fee basis by the Austin Scottish Rite Theater, to perform treatments and procedures as deemed necessary; and, release all medical or hospital records to Austin Scottish Rite Theater from existing hospital and medical records; and, release all medical and hospital records possessed by Scottish Rite Theater to other physicians, nurses, hospitals and their authorized personnel; and allow Austin Scottish Rite Theater personnel, EMTs, or other medical professionals to transport my child in the event of an emergency. All releases and authorizations are for performance of treatment, procedures and medications as deemed necessary for my applicant. 

Dismissal Policy

Our founding principles of safety, well-being, and happiness will be applied to the determination of dismissal, as maintaining a safe environment is our first priority. By reviewing and signing the Dismissal Policy form, you acknowledge your understanding of this policy. It is the Austin Scottish Rite Theater Camp policy to dismiss a camper in the following circumstances: Upon direct orders of a physician; When camp administration determines that the camper needs services and supervision beyond those provided by our camp and our staff; When the camper exhibits any of the following behaviors or conditions: Aggressive or threatening behaviors, Refusal of prescribed medications, Non-compliant behavior, Inappropriate sexual behavior, Throwing objects, Biting, scratching, kicking, fighting, Destruction of property, Incontinence of bowel and/or bladder, Inability to complete self care tasks (bathing, toileting, feeding, etc.). Should a camper be dismissed for any of the above reasons, or due to the request for discharge by the camper, family or legal guardian, tuition will not be refunded. 

 

Liability Release

The undersigned parties, being the parent of the Applicant, have enrolled the named child or children in Austin Scottish Rite Theater Summer Camp. The camp session will take place primarily in the Austin Scottish Rite Theater. Activities may also include, but are not limited to, stage swordplay and other physical activities both inside and outside the building. Campers will need to wear clothing and shoes that are appropriate for these activities. It will be important for campers to follow the directions and instructions given them by staff so they will be safe and have an enjoyable camp experience. No alcoholic beverages or other types of drugs are permitted under any circumstances. IN CONSIDERATION OF THE SERVICES PROVIDED, BY SIGNING THIS LIABILITY RELEASE, I AGREE TO BE LIMITED TO THE TERMS OF THIS RELEASE, AND I UNDERSTAND THAT I AM HOLDING AUSTIN SCOTTISH RITE THEATER, ITS OFFICERS, DIRECTORS, EMPLOYEES, INSTRUCTORS, VOLUNTEERS, AND OTHER REPRESENTATIVES HARMLESS AND FREE FROM LIABILITY FOR ANY AND ALL ACTS AND OMISSIONS FOR DAMAGES, INJURIES OR ANY CLAIM OF LIABILITY THAT MIGHT ARISE AS A RESULT OF INJURY TO MY CHILD AS A RESULT OF HIS OR HER PARTICIPATION IN THE SCOTTISH RITE THEATER SUMMER CAMPS.
